Menopoz Semptomlarının Şiddetinin Duygusal Yeme ile İlişkisi

Abstract

Amaç: Duygusal yeme, fizyolojik açlıktan bağımsız olarak bireyin duygusal değişimlere yanıt olarak sergilediği yeme davranışı olup obezite ve bozulmuş yeme davranışlarıyla ilişkilidir. Menopoz, kadının hayatında biyopsikososyal değişimlerin yaşandığı ve çeşitli problemlerin yaşandığı bir dönemdir. Bu çalışmada menopoz semptomlarının şiddeti ile duygusal yeme davranışı arasındaki ilişkinin incelenmesi amaçlandı. Gereç ve Yöntem: Bu tek merkezli, kesitsel çalışma, üçüncü basamak bir sağlık kuruluşunda yer alan Aile Hekimliği polikliniğinde Mayıs–Eylül 2025 tarihleri arasında yürütüldü. STRAW+10 evreleme sistemine göre geç menopozal geçiş ve postmenopozal dönemde olan 211 gönüllü kadın çalışmaya dahil edildi. Katılımcıların yaş, medeni durumu, çalışma durumu, eğitim durumu, sigara kullanma durumu, gebelik sayısı, ek hastalıkları, boy ve kiloları kaydedildi. Ayrıca, 11 sorudan oluşan 'Menopoz semptomlarını değerlendirme ölçeği' (MSDÖ) ve 30 sorudan oluşan ''Türkçe duygusal yeme ölçeği''(TDYÖ) uygulandı. Bulgular: TDYÖ toplam puanı için oluşturulan yapısal eşitlik modeli sonucunda MSDÖ somatik alt boyut puanının duygusal yeme ile negatif, psikolojik ve ürogenital alt boyut puanlarının ise pozitif ilişkili olduğu tespit edildi (sırasıyla; B=−0.887, p=0.047, B=1.320, p=0.001, B=2.464, p<0.001). TDYÖ toplam puanı ile ilişkili diğer faktörler eğitim durumu, medeni durum, çalışma durumu, menopozal evre ve beden kitle indeksi idi (sırasıyla; B=−10.523, p=0.003, B=−13.057, p<0.001, B=6.708, p=0.015, B=8.722, p=0.002). Ayrıca evli olmanın, sigara kullanıp bırakmış olmanın ve psikiyatrik hastalık varlığının MSDÖ ürogenital semptom puanları üzerinden TDYÖ toplam puanlarına dolaylı etkisi olduğu görüldü. Sonuç: Bu çalışmada geç perimenopoz ve postmenopoz dönemindeki kadınlarda menopoz semptomlarının şiddeti ile duygusal yeme davranışı arasında anlamlı ilişkiler bulunduğu görülmüştür. Menopoz semptomlarının alt boyutları incelendiğinde, psikolojik ve ürogenital semptomların duygusal yeme davranışını artırdığı, somatik semptomların ise ters yönde bir ilişki gösterdiği saptanmıştır. Menopoz dönemindeki kadınlar, hormonal dalgalanmalar ve artan semptom yükü nedeniyle duygusal yeme açısından özel bir risk grubudur. Bu bulgular, menopoz dönemindeki kadınların değerlendirilmesinin, yalnızca somatik semptomlarla sınırlı kalmaması; psikolojik belirtiler, ürogenital yakınmalar ve yeme davranışlarının bütüncül bir yaklaşımla ele alınması gerekliliğini göstermektedir.
Aim: Emotional eating is an eating behavior exhibited in response to emotional changes, independent of physiological hunger, and is associated with obesity and disordered eating behaviors. Menopause is a period in a woman's life when biopsychosocial changes occur and various problems arise. This study aimed to examine the relationship between the severity of menopausal symptoms and emotional eating behavior. Materials and Methods: This single-center, cross-sectional study was conducted in a Family Medicine outpatient clinic of a tertiary healthcare facility between May and September 2025. Two hundred eleven volunteer women in the late menopausal transition and postmenopausal periods, according to the STRAW+10 staging system, were included in the study. Participants' age, marital status, employment status, education level, smoking status, number of pregnancies, comorbidities, height, and weight were recorded. In addition, the Menopausal Rating Scale (MRS), consisting of 11 questions, and the Turkish Emotional Eating Scale (TEES), consisting of 30 questions, were administered. Results: The structural equation model created for the total score of the TEES revealed that the somatic subscale score of the MRS was negatively correlated with emotional eating, while the psychological and urogenital subscale scores were positively correlated (respectively; B=−0.887, p=0.047, B=1.320, p=0.001, B=2.464, p<0.001). Other factors associated with the total score of the Emotional Eating Scale were education level, marital status, employment status, menopausal stage, and body mass index (respectively; B=−10.523, p=0.003, B=−13.057, p<0.001, B=6.708, p=0.015, B=8.722, p=0.002). Additionally, being married, having quit smoking, and the presence of psychiatric illness were found to have an indirect effect on the total TEES scores via the MRS urogenital symptom scores. Conclusion: This study found significant correlations between the severity of menopausal symptoms and emotional eating behavior in women in the late perimenopausal and postmenopausal periods. When examining the sub-dimensions of menopausal symptoms, it was determined that psychological and urogenital symptoms increased emotional eating behavior, while somatic symptoms showed an inverse relationship. Women in menopause are a particular risk group for emotional eating due to hormonal fluctuations and increased symptom burden. These findings indicate that the assessment of women in menopause should not be limited to somatic symptoms alone; a holistic approach encompassing psychological symptoms, urogenital complaints, and eating behaviors is necessary.
