Abstract
Amaç: Bu çalışmada Trabzon il merkezinde lise çağındaki adölesanlarda primer baş ağrısı sıklığını belirlemek amaçlandı. Gereç ve Yöntem: Kesitsel desende gözlemsel tasarıma sahip bu toplum tabanlı prevalans çalışmasında, tabakalı rastgele örnekleme yöntemiyle seçilen Trabzon il merkezinde öğrenim gören lise öğrencilerine anket uygulandı. Migren ve gerilim tipi baş ağrısı (GTBA) tanılarını saptamak ve klinik özellikleri değerlendirmek için, Uluslararası Baş Ağrısı Bozuklukları Sınıflaması (ICHD-3) kriterleri dikkate alınarak geliştirilen anket formu kullanıldı. Tanımlayıcı istatistiklerin yanı sıra, bağımsız gruplarda T testi, Mann-Whitney U testi ve Ki-kare testleri uygulandı. İstatistiksel anlamlılık düzeyi p<0,05 olarak alındı. Bulgular: Çalışmaya katılan %52,1'i (n=424) kadın toplam 814 kişinin yaş ortalaması 15,7±1,1 olup, %73,5'inde (n=598) primer baş ağrısı saptandı. Katılımcıların %85,1'inin (n=693) son altı ayda baş ağrısı yaşadığı belirlendi. Primer baş ağrılarının %66,4'ü (n=397) migren, %33,6'sı (n=201) GTA olarak sınıflandı. Primer baş ağrısı olanların %68,2'sinin (n=408) günlük aktivitelerinin etkilendiği, %19,4'ünün (n=116) son üç ay içinde baş ağrısı nedeniyle okula devamsızlık yaptığı görüldü. Bu gruptakilerin %89,5'i (n=535) en az bir tetikleyici bildirmiş olup, en sık bildirilen %77,9 (n=417) ile uykusuzluk idi. Baş ağrısı atak sıklığı arttıkça, uyku süresinin azaldığı gözlendi (p<0,001). Migreni olanlarda, GTA olanlara kıyasla günlük aktivitelerin etkilenme oranı anlamlı derecede yüksek idi (sırasıyla %73,8, %57,2, p<0,001). Sonuç: Trabzon il merkezinde lise çağında baş ağrısı prevalansı oldukça yüksektir ve migren en sık görülen primer baş ağrısı tipidir. Baş ağrısının, özellikle migrende, günlük işlevsellik ve okul devamsızlığı üzerinde ciddi olumsuz etkileri bulunmaktadır. Uykusuzluk ile güçlü ilişkisi, bu yaş grubunda koruyucu yaklaşımların ve yaşam tarzı düzenlemelerinin önemini ortaya koymaktadır.
Aim: The aim of this study is to determine the prevalence of primary headaches among high school-aged adolescents in the city center of Trabzon. Materials and Methods: This cross-sectional, observational, community-based prevalence study was conducted by administering a questionnaire to high school students in Trabzon selected via stratified random sampling. A questionnaire form developed by the researchers in accordance with the International Classification of Headache Disorders (ICHD-3) criteria was used to diagnose migraine and tension-type headache (TTH) and to evaluate their clinical features. In addition to descriptive statistics, independent samples T-test, Mann-Whitney U test, and Chi-square tests were applied. The statistical significance level was accepted as p<0.05. Results: The study included a total of 814 participants, 52.1% (n=424) of whom were women, with an average age of 15.7±1.1 years. Primary headache was detected in 73.5% (n=598) of these participants. It was determined that 85.1% (n=693) of the participants had experienced a headache within the last six months. Of the primary headaches, 66.4% (n=397) were classified as migraine and 33.6% (n=201) as TTH. Among those with primary headache, 68.2% (n=408) reported that their daily activities were affected, and 19.4% (n=116) reported school absenteeism due to headache in the last three months. In this group, 89.5% (n=535) reported at least one trigger, with the most frequently reported trigger being insomnia (77.9%, n=417). As headache attack frequency increased, sleep duration was observed to decrease (p<0.001). The rate of daily activity impairment was significantly higher in those with migraine compared to those with TTH (73.8% vs. 57.2%, respectively; p<0.001). Conclusion: The prevalence of headache among high school students in Trabzon is quite high, with migraine being the most common type of primary headache. Headache, especially migraine, has serious negative effects on daily functionality and school attendance. Its strong association with insomnia highlights the importance of preventive approaches and lifestyle modifications in this age group.
